Purchase a replacement Founder's Award certificate with an updated name. In the order process, please provide the name listed on the original certificate, updated name for new certificate, council name, lodge name, and date awarded.
Name changes are common in our society and occur for many reasons, including (but not limited to) religious conversion, marriage and divorce, and to be in alignment with a person’s gender identity. In keeping with our organization’s commitment to inclusion, Order of the Arrow members - current and alumni - may submit this name change form for any reason to request replacement recognition items.
Replacement credentials are in the current format with the current National Director and National Chair signature. We apologize that we are unable to provide credentials other than the current year's format or include the Lodge Chief, Lodge Adviser, or Scout Executive signatures from the issuing lodge.
